Most manufacturers still run a reactive quality model, and it looks completely normal from the inside.
One operator performs the task. A second operator inspects it. Everyone accepts this as the cost of doing things properly.
Look at what it actually costs.
It doubles labor on every critical step. It introduces subjectivity, because a human inspecting is a human having a good or a bad day.
