Everyone argues about which model to use. Almost nobody talks about how the documents get in. That is backwards. Garbage in, confident nonsense out. A weak model on clean, well-labeled information beats a frontier model drowning in duplicates and stale drafts every time. Here is the one that gets companies. The same policy exists in nine slightly different versions across your tools. Feed all nine to the AI and it averages them into an answer that was never true, then quotes it to a customer with total confidence. Nobody wrote that policy. The machine did, by averaging your mess.
