Remecare's platform enables care teams to provide continuous care in the hospital and at home through personalised care pathways and patient reported outcomes. The platform gives clinicians access to data and information needed to coordinate care across care actors. It is built by Remedus and focuses on transmural care across specialties including oncology, neurology, heart failure, IV antibiotics, rheumatology, and pneumology.
