It is the moment a critical secret becomes permanently inaccessible because the human layer around it was never governed.
A vault that nobody can open is not secure infrastructure. It is a liability with a very strong door.
We built an entire industry around keeping secrets safe from the wrong hands. We built almost nothing to ensure they remain accessible to the right ones.
That asymmetry is the actual risk. And it compounds silently, across every organisation, every system, every personnel change, every succession event that happens without a clean handover.
Security without governance is not a solved problem.
It is a delayed one.
