A cubitus valgus deformity of the distal humerus rarely comes down to a single angle. A rotational component is usually present alongside the coronal malalignment, and both have to close together for the elbow to sit right. Correct only the angle, and the rotation is left to freehand judgment.
This case combined a 20° medial closing correction with a 12° derotation, planned and executed as one single correction.
The patient-specific guide system was built around four K-wires placed before any cut. They become parallel only once both the angular closing and the rotation have been applied together, in the exact position planned preoperatively, a direct, visible signal that the correction is complete, with no navigation or fluoroscopic measurement required.
This case was performed by Dr. Robert Elbaum at Chirec Delta, Brussels, Belgium.
In this context, the K-wires are not just fixation. They are the proof that the plan was reproduced.
