Overview

QuotedGreg Shaw@greg-shaw-760a0334On Tuesday last week I performed LaserSAFE / complete urethral preservation (CUP) for three patients during robotic radical prostatectomy (RARP) on a single full day NHS operating list at University College London Hospital (UCLH. All three patients would not have been offered full nerve sparing without the availability of intraoperative margin evaluation. All three patients came off the operating table having had full bilateral intrafascial nerve sparing. This was a genuinely satisfying experience. The aim is simple: maximise cancer control while protecting the things that matter most to patients afterwards — urinary continence, erectile function, and a rapid return to normal life. CUP preserves the full length of the urethra during RARP, which is associated with improved early continence recovery and higher rates of pad-free status, without compromising oncological outcomes. https://lnkd.in/exCBRFC9 LaserSAFE uses the Histolog scanner for intra-operative confocal microscopy assessment of surgical margins in minutes, supporting more confident, tailored nerve-sparing surgery. https://lnkd.in/eZidYPNm Importantly, all three cases were completed without any additional theatre time compared with standard RARP. The histopathologists were able to examine the images from their offices without the prostate having to be transported to the laboratory — making LaserSAFE a practical, scalable alternative to NeuroSAFE frozen section evaluation, for which the excised prostate has to be transported to the lab and which adds almost an hour onto each case, rendering NeuroSAFE impractical for much of the NHS. I am proud that UCLH is currently the only NHS hospital offering LaserSAFE, excited that Royal Surrey will be starting soon and optimistic that prostate cancer surgeons will refer NHS patients who would not ordinarily be offered full nerve sparing, so that intra-operative margin evaluation can be made available to all who need it, resulting in more extensive nerve sparing and the improved erectile function recovery that results.
