In september is de eerste Histolog® Scanner in Nederland geïnstalleerd in het St. Antonius Ziekenhuis. De eerste procedures zijn inmiddels uitgevoerd onder begeleiding van onze collega's Sandra Berkenbosch, Bernard Jordaan en Tom Engels. Dit markeert een belangrijke stap vooruit in de toepassing van Intra Operatieve Marge Assessment (IOMA) tijdens zenuwsparende radicale prostatectomieën. Bij een zenuwsparende prostatectomie staan chirurgen voor een cruciale uitdaging: het volledig verwijderen van de tumor terwijl de omliggende zenuwstructuren zoveel mogelijk behouden blijven. Juist binnen dit delicate evenwicht kan intraoperatieve margestatusbeoordeling een belangrijke meerwaarde bieden. Met de Histolog Scanner kan tijdens de ingreep het oppervlak van het verwijderde weefsel worden gevisualiseerd. Dankzij confocale microscopie zijn binnen enkele minuten beelden met een hoge resolutie beschikbaar, waarbij slechts minimale preparatie van het weefsel nodig is. Deze beelden kunnen vervolgens door een patholoog worden beoordeeld, zelfs direct op afstand. Belangrijk is dat het verwijderde weefsel volledig intact blijft voor de definitieve pathologische analyse. IOMA kan chirurgen tijdens de operatie aanvullende informatie verschaffen over de chirurgische snijvlakken. Chirurgen gebruiken deze informatie, samen met hun klinische expertise en oordeel, om beslissingen tijdens de ingreep te ondersteunen. Het uiteindelijke doel blijft een volledige verwijdering van de tumor, met maximaal behoud van gezonde zenuwstructuren.
