Your systems match by name. That's the bug.
A bug I fixed in my own reporting last week is sitting in a lot of small businesses right now.
The system looked something up by its name instead of its unique ID.
It found something with an almost identical name. It picked the wrong one. And it never once threw an error.
If you run a small team, you probably have this bug somewhere:
Two suppliers saved as "Martin & Co" and "Martin and Co" — invoices landing on the wrong account for months.
The same customer sitting three times in your CRM because someone typed the name slightly differently each time.
Products matched by label instead of SKU, so your stock report is quietly off.
A lead dropped as a duplicate because this time the company name had "SA" at the end.
None of these crash. That's exactly the problem.
A broken process gets fixed on Monday morning. Someone shouts, someone opens a ticket.
A process that is quietly, plausibly wrong just keeps running. It gets trusted. People start making decisions on top of it.
This is the part of automation nobody warns you about.
When I map a workflow, half the work isn't deciding where to add AI. It's:
What is the unique identifier here, and does the system actually use it?
What happens when it can't find a match?
Who sees the number before someone acts on it?
The rule I apply everywhere now: if the system can't identify something with certainty, it stops and asks a human. It never guesses quietly.
AI prepares. Humans approve.
